Coumadin Clinic

The Coumadin clinic was started to enhance the care of our patients taking Coumadin. By following the protocol established by our Physicians we are better able to help you as patients maintain therapeutic levels in your blood test.

Coumadin is an anticoagulant. It helps reduce clots from forming in the blood. Your doctor may have placed you on Coumadin for various reasons. Coumadin is used to prevent and treat blood clots in the legs and lungs, and blood clots associated with atrial fibrillation, and heart-valve replacement. You may also be taking Coumadin for a history of stroke or a knee or hip replacement.

It is very important to have your blood tests done as ordered by your doctor to ensure you are on the correct dosage of Coumadin. Listed below are therapeutic ranges for various diagnoses.

Stroke, blood clot, atrial fib., knee and hip replacements:
Therapeutic Range between 2.0-3.0

Valve replacement:
Therapeutic Range between 2.5-3.5
