New Patient Information

New patients are required to bring along the following:

• Drivers Licence, Student Identification or Picture ID
• All Insurance Cards including Medicare and Medicaid along with card holders date of birth and address
• Any Prescription Medications or Supplements (including herbs & natural remedies) in their original bottles 
• Insurance Copay (if applicable)
• Medicaid Copay (if applicable)
• If Under the Age of 18, a Legal Guardian Must be Present

Please arrive 15 minutes prior to your appointment for registration.

Prior to your appointment at Great Plains Clinic, P.C., we request that all previous medical records be sent to 33 Ninth Street West, Dickinson, North Dakota 58601 or faxed to 701-483-5018.

If you have an insurance plan that requires a copay (i.e. Blue Cross) we will collect this from the patient at the time of registration. You will be billed for all additional services as soon as your insurance plan has paid Great Plains Clinic, P.C.

• New Patient Visit    $126.00
• Established Patient Visit    $73.00
• DOT Physical   $122.00
