Third Party Liability (TPL) Visits.

If you have been injured, become ill or involved in a Motor Vehicle Accident (MVA) and a third party is responsible for the charges; please note that you are responsible for these charges. Great Plains Clinic, P.C. will not file these claims to a third party, nor will we wait for payment from them. We will provide you with a bill (HCFA 1500 claim form) and you may file your claim with that third party. Upon receipt of your statement, you are responsible for making payment in full for services rendered.

It is important that patients with insurance coverage realize that professional services are rendered to a person, not an insurance company. Therefore, the insurance company is responsible to the patient and the patient is responsible to Great Plains Clinic P.C.
